New to Chart Hound?
Follow the steps below to get started with Chart Hound. If you need help, scroll down to contact our support team.
Contact SupportDownload the App
Chart Hound is available on iOS, Android, and the web dashboard. Download from the App Store or Google Play, or log in directly from your browser.
Sign In
Open the app and sign in with the credentials provided by your clinic administrator. You can use email/password, Google Sign-In, or Apple Sign-In.
Accept AI Data Consent
On your first recording, you'll see a transparency disclosure explaining how your data is processed. Review and accept to enable recording. You can review or revoke consent anytime in Account settings.
Start Recording
Tap the record button on the dashboard. Speak naturally as you examine the patient. Chart Hound will transcribe and structure your notes into a professional SOAP format.
Review & Share
Review your completed SOAP note, make any edits, and share a pet parent-friendly summary via the Report Card feature.
How to Record
Tips for Best Results
- Speak naturally — talk as you would to a colleague. No special commands needed.
- Include patient details — mention species, breed, age, and patient name for accurate notes.
- Quiet environment helps — minimize background noise for better transcription accuracy.
- Up to 20 minutes — each recording session supports up to 20 minutes of continuous audio.
File Upload
You can also upload pre-recorded audio files by tapping the drop zone on the recording screen. Supported formats include MP3, M4A, WAV, and other common audio formats.
Offline support: If you lose connectivity during a recording, your audio is encrypted and queued locally. It will automatically upload when you're back online.
Understanding Your SOAP Notes
S — Subjective
History reported by the owner: presenting complaint, duration of symptoms, appetite, behavior changes, and relevant medical history.
O — Objective
Physical exam findings: vitals, body condition score, auscultation, palpation results, and any diagnostic data from labs or imaging.
A — Assessment
Your clinical assessment: differential diagnoses, confirmed diagnoses, and overall patient condition evaluation.
P — Plan
Treatment plan: medications prescribed, procedures performed or scheduled, follow-up instructions, and client communication notes.
Editing Your Notes
Every AI-generated note is fully editable. Tap any section to modify the text directly. Your edits are saved with an audit trail so you always have a record of changes. Notes can also be viewed in four tabs: SOAP, Summary, Dental, and Raw transcription.
Managing Your Account
How Accounts Work
Chart Hound accounts are provisioned by your clinic administrator or by our support team. If you're part of a clinic, your admin will invite you and assign your role. Individual practitioners can contact our team to get set up.
Clinic Membership
Once your admin adds you to a clinic, you'll see shared patients, templates, and clinic settings. Your role (admin, veterinarian, or technician) determines which features are available to you.
Account Settings
Access your account settings from the Account tab. Here you can update your professional title (e.g. DVM, Vet Tech), change your password, review AI data consent, and manage your learning profile.
AI Data Consent
Chart Hound requires your explicit consent before processing audio with AI. You can review the full data processing disclosure and revoke consent at any time from the Account tab. Revoking consent will pause recording capabilities but will not affect your existing notes.
Deleting Your Account
You can permanently delete your account and all associated data from the Account tab. This action cannot be undone. You may also request account deletion by emailing support@charthound.ai.
